In October 2025, Deloitte Australia made global headlines — and not for the right reasons. A government-commissioned report the firm delivered was found to contain AI-generated errors, including fabricated citations, misquoted legal rulings, and references to academic papers that didn’t exist (Associated Press, 2025; Financial Times, 2025).


After the discovery, Deloitte was forced to refund part of the AU$440,000 contract and reissue a corrected version (TechRadar, 2025). What began as an efficiency play using generative AI quickly became a reputational setback — one that could have been prevented with better AI governance and employee training.

Deloitte later admitted that its internal quality checks failed to catch the fabricated content before delivery. The company also hadn’t disclosed that AI tools (in this case, Microsoft’s Azure OpenAI GPT-4o) were used in drafting the report. Only after public scrutiny did Deloitte update the report with an acknowledgment of AI use.


In a statement to the press, Deloitte leaders conceded that “oversight was not followed.” That oversight — or lack thereof — illustrates a broader problem many organizations face today: they are embracing AI faster than they are training their people to use it responsibly.
